Cause of cardiomegaly
In most of the cases, there is no apparent cause. It is due to the result of damaged myocardium. Infectious pathogens or metabolic endotoxins are the principal causes of myocardial injury. Cardiomegaly could have been caused on by an earlier myocardial infarction attack.

Signs & Symptoms of cardiomegaly
The symptoms of an enlarged heart can vary from person to person. These develop insidiously. It can be asymptomatic too and may not be initially significant enough to affect the quality of life of the patient. Some of the main symptoms include:
Tiredness & fainting (syncope)
Dizziness & light-headedness
Shortness of breath
Swelling in legs
Palpitations (fluttering in the chest due to abnormal heart rhythms)
Chest pain (angina, but only if ischemic heart disease is present)

A patient suffering from this condition may be characterized by elevated jugular venous pressure, low pulse pressure, or pulmonary edema. Signs of tricuspid and mitral regurgitation may also be present.

The chances for the formation of a thrombus (blood clot) are comparatively high in DCM patients as the blood flow velocity is slowed down. This may again increase the chances of embolism. It may end up in pulmonary embolism (lungs), stroke (cerebral emboli), renal emboli or peripheral emboli (limbs).

Blood tests
Chest X-ray – generalized heart enlargement can be seen. Additionally, pleural effusion brought on by pulmonary venous hypertension is also visible.
On an electrocardiogram, sinus tachycardia, atrial fibrillation, arrhythmia, left atrial enlargement, and other abnormalities can be seen.
An echocardiogram reveals left ventricular dilatation and a decreased ejection fraction.
